Re-Bath Customer Referral Form

HOW TO ENTER:
To enter, you must be a previous or existing Re-Bath customer who has sent Re-Bath a friend, colleague or family member as a valid referral during the “Contest Period” which is defined below. For each “Valid Referral” a person makes they will receive one (1) entry into the Contest without any maximum amount of entries. Limit one contest entry per referral, per contest period and limit one entry per person, unless the person submits several “Valid Referrals.”
A “Valid Referral” is defined as a new customer who has requested a FREE In-Home Estimate and has purchased a Re-Bath bathroom remodeling project with “Re-Bath the South.”
“Re-Bath the South” is defined as a group of Re-Bath franchises under the same ownership: Re-Bath of Mobile, Re-Bath of Central Alabama, Re-Bath of Savannah, Re-Bath of Northwest Florida, Re-Bath of Tallahassee and Re-Bath of Jacksonville.
Referrals can be submitted by several methods: 1. Online, by visiting our website, www.rebaththesouth.com, clicking the ‘Refer A Friend’ link, and then completing and submitting all required information on the entry page. 2. Referral Form, to be completed and supplied by the Re-Bath sales consultant at the time of the estimate. Referral Forms must have the name and contact information (address and phone number) of the referred customer and the name and contact information of the previous Re-Bath customer who made the referral in order to be considered a valid referral entry. Forms that are not completed with all the necessary information will not be entered into the contest.

THE CONTEST:
The Contest runs from September 1, 2008 - August 31, 2009 (the “Contest Period”). Re-Bath the South reserves the right to terminate the contest if events beyond its reasonable control make continuation not feasible. Re-Bath the South also reserves the right to terminate, suspend or amend this Contest should a virus, bug, or computer, electronic or system malfunction, or another cause beyond Re-Bath the South’s control corrupt the administration, security or proper play of this Contest. This promotion is the sole responsibility of Re-Bath the South.

THE PRIZE:
There are five (5) prizes to be awarded structured as follows:

Double occupancy. Travel Insurance is not included, but can be added at the winner’s expense. Any other expenses incurred are the responsibility of the winner including but not limited to gratuities, telephone charges, hotel mini bar, etc. unless otherwise noted. The cost of a passport is not included, but is required for the trip to Jamaica. If the contest winner does not possess a passport at the time of the drawing, the contest winner will be required to choose between the trip to Las Vegas or Wine Country. The Prize must be used during the following period: September 1, 2009 to March 31, 2010, excluding the period December 19, 2009 to January 4, 2010.
All vacations are for the winning person and up to one (1) guest and is subject to availability. Vacations may not to be sold, traded, transferred, refunded, redeemed for cash value or rescheduled to dates other than as set forth above.
The Prize must be accepted as awarded, without substitutions. Any additional costs incurred in collecting and using the Prize are each winner’s responsibility. Maximum one Prize to contest winner. Re-Bath the South reserves the right to substitute prizes of equal or greater value.

THE ENTRIES:
All entries become the property of Re-Bath the South and none will be returned; Re-Bath the South assumes no responsibility for lost, delayed, destroyed or misdirected entries. Re-Bath the South will contact only those persons whose names are drawn.
THE DRAWING:
On September 1, 2009, at approximately 2 P.M. (CST) one (1) entry for the Grand Prize and Second Prizes will be randomly drawn at 273 Azalea Road, Suite 2-204, Mobile, Alabama, from all eligible entries received during the Contest Period. The chances of being selected depend on the total number of eligible entries received during the Contest Period. The person whose name is drawn first is eligible for the Grand Prize. The next four names drawn will be considered second prize winners in the order they are drawn. They will also be considered alternate winners for the Grand Prize, in case a selected entrant cannot be contacted. Ten additional names (or reasonable number as determined by Re-Bath the South or its agent) will be drawn as backups in case the selected entrants for any of the three prizes cannot be contacted. Each entrant will be given a two (2) week period from the draw date to respond to Re-Bath the South before the next alternate or backup name is contacted.

THE WINNER(S):
All decisions of Re-Bath the South and the contest judging organization representatives are final. Then, after Re-Bath the South and the judging organization verifies that you have complied with these rules, the winners must sign Re-Bath the South’s standard release form before being entitled to receive the Prize. That form also contains the consent of each winner and/or the travelling companions (for all prizes) to the use of their names, addresses and/or photographs in advertising or other publicity by Re-Bath the South and/or its agencies, without any payment or other compensation.
The names of the winners may be obtained by writing Re-Bath the South Referral Contest Giveaway, 273 Azalea Road, Suite 2-204, Mobile, Alabama 36609, and will be posted on our website at www.rebaththesouth.com.

WHO IS ELIGIBLE:
The Contest is open to any individual who is a previous or existing Re-Bath customer who has made a “Valid Referral(s)” during the “Contest Period” and is a resident of The United States of America and who has reached the age of majority. Exceptions include employees of Re-Bath or its affiliated companies, and persons domiciled with any such person, or a member of the immediate family of any such person (spouses, parents, children, grandparents, grandchildren, siblings, by blood or adoption, and spouses of same). By submitting your complete name and e-mail address, your consent to the collection and use of this information in accordance with the Privacy Commitment.
